Response: The app helps patients track their blood glucose, activity, meals, and medicines—the four cornerstones of diabetes care. It also includes helpful information about healthy eating and lifestyle choices from the Novo Nordisk Cornerstones4Care® diabetes support program.

The app syncs with the majority of diabetes devices available, including blood glucose meters, insulin pumps, and continuous glucose monitors, as well as many fitness trackers such as Fitbit and Moves. If patients use more than one device, they can see all their data together. The app also lets patients set reminders, and puts educational content right at their fingertips.

Response: The app is available for patients to download on the iTunes App Store (for iPhones) or Google Play (for Android devices). There is no cost to download or use the app. Patients can also download the C4C app when they enroll into Cornerstones4Care.com. Users will get a Welcome Kit that contains everything they need to sync their current devices with the app.
