Salvage Prostatectomy Helps Some Patients After Prostate Cancer Recurs Post Radiation

MedicalResearch.com: What is the background for this study? What are the main findings?

Dr. Pokala: The main purpose of the study was to determine survival outcome following salvage prostatectomy in men that fail radiation therapy. Radiation and surgery are the main modalities utilized to treat localized prostate cancer. When patients fail radiation treatment, traditionally, only hormonal treatment was offered. With refinements in surgical techniques, a select few of these patients that have recurrence after radiation may benefit with salvage surgery. Salvage prostatectomy is a complex procedure because prior radiation makes this procedure tenuous, but this procedure is offered in most major tertiary medical centers.

MedicalResearch.com: What should clinicians and patients take away from your report?

Dr. Pokala: This study was performed utilizing the Surveillance Epidemiology and End results (SEER) database and included patients that underwent salvage radical prostatectomy after having received radiation to treat their prostate cancer. The study demonstrated that excellent prostate cancer specific survival can be achieved in these cohort of patients. The cancer specific survival was as high as 88.6 percent at 10 years and about 73 percent at 20 years after salvage prostatectomy. This demonstrates that potentially curative surgery is an option in select patients that develop recurrence after radiation.

Physicians taking care of prostate cancer patients should discuss salvage prostatectomy as an option when they recur after radiation. This is a complex procedure and has a higher complication rate compared to primary radical prostatectomy. We do not have the information of complications following salvage prostatectomy in the SEER database and this was not studied.

MedicalResearch.com: What recommendations do you have for future research as a result of this study?

Dr. Pokala: Further large single or multi-institutional studies are required to confirm these findings from the national database, and further studies looking specifically into the complication rates of this complex procedure are required.
