Medical Research: What is the background for this study? What are the main findings?
Dr. Olbers: Until now there has been no consensus regarding preferred bariatric procedure for patients with a body mass index (BMI) above 50 kg/m2. We report on the 5-year outcomes from a randomized clinical trial of gastric bypass and duodenal switch published online by JAMA Surgery on February 4th.
Medical Research: What should clinicians and patients take away from your report?
Dr. Olbers: Duodenal switch resulted in more weight loss and greater improvements in several cardio-metabolic risk factors compared with gastric bypass when treating patients with a very high weight. However, duodenal switch was associated with more surgical, nutritional and gastrointestinal adverse effects making us recommend caution in the use of this procedure.
Medical Research: What recommendations do you have for future research as a result of this study?
Dr. Olbers: Based on our findings we conclude that duodenal switch should be used with caution owing to a relatively high rate of long-term adverse effects in comparison to gastric bypass yet the greater weight loss.
