Dr. Grieger: The study aimed to identify associations between maternal dietary patterns in the 12 months before conception on fetal growth and preterm delivery. We report that a one standard deviation increase in the scores on the high-protein/fruit pattern was associated with decreased likelihood for preterm birth, whereas a one standard deviation increase on the high-fat/sugar/takeaway pattern was associated with increased likelihood for preterm birth as well as shorter gestation and birth length. Dr Bulló: There is some evidence that different dietary forms of vitamin K could exert varying effects on health, however no study to date has simultaneously evaluated the potential effects of the main vitamin K forms on cancer and cardiovascular mortality. We conducted a prospective, epidemiologic study involving 7,216 elderly subjects at high cardiovascular risk who were followed for about 5 years.The results of the present study show, for the first time that an increase in dietary intake of both forms of vitamin K is related to a lower risk of cardiovascular mortality, cancer mortality or all-cause mortality. Dr. Lu Qi: In this study, we for the first time provide reproducible evidence from three large cohort studies to show that the association between regular consumption of fried foods and higher BMI was particularly pronounced among people with a greater genetic predisposition to obesity. On the other hand, the adverse genetic effects on BMI were also amplified by consuming more fried foods, the effects among those who ate fried foods more than four times a week was about twice as large compared with those who ate them less than once a week. Dr. Chowdhury: Total saturated fatty acid, whether measured as a dietary intake variable or in the bloodstream as a biomarker, was not associated with coronary disease risk in combining all available prospective observational studies. Similarly, there were non-significant overall associations in the prospective studies that involved assessments of total monounsaturated fatty acids, long-chain omega-3 and omega-6 polyunsaturated fatty acids. However, we found diversity in the observational associations between specific circulating long-chain omega-3 and omega-6 fatty acids with coronary risk, with some evidence that circulating levels of eicosapentaenoic and docosahexaenoic acids (ie, the two main types of long-chain omega-3 polyunsaturated fatty acids), and arachidonic acid are each associated with lower coronary risk. Similarly, within saturated fatty acids, there were positive, however, non-significant associations observed for circulating blood composition of palmitic and stearic acids (found largely in palm oil and animal fats, respectively), whereas circulating margaric acid (a milk fat) had a significant inverse association. Additionally, when we investigated the randomised controlled trials that reported on the effects of omega-3 and omega-6 fatty acids on reducing coronary outcomes, there was no significant overall association observed. Dr. Risérus: It has previously been shown in animal studies that overfeeding polyunsaturated fat causes less fat accumulation than saturated fats, but this study is the first to suggest that this could be true also in humans. Overeating saturated fats caused significantly more accumulation of fat in the liver and intra-abdominally, as compared with overeating polyunsaturated fats from. This study suggests it does matter where the excess calories come from when we gain weight. If a high-caloric diet contains large amounts of saturated fats it seems to switch on some genes that may promote abdominal fat storage and insulin resistance, and thereby result in a more unfavorable fat storage. In contrast, such effects were not seen if the diet was lower in saturated fats but higher in polyunsaturated fats from non-tropical vegetable oils. This study also suggests a novel contributing factor regarding the tendency of some individuals to accumulate fat in the liver and abdomen, i.e. in some people excessive amounts of saturated fat in combination with sugars might induce more fat in their livers and a propensity towards abdominal visceral fat accumulation. Answer: We know fruits and vegetables seem to be particularly important for prevention of  heart disease and diabetes but what constituent may responsible for these benefits is unclear. These foods contain powerful bioactive compounds called flavonoids and in lab and animal experiments we know that flavonoids can reduce inflammation, improve BP, keep our arteries healthy and flexible, improve blood flow and reduce cholesterol levels. Our previous work had shown that a higher level of one class of flavonoids, the anthocyanins, responsible for the brilliant red/blue colours in fruits and other plant foods/products, could reduce the risk of developing type 2 diabetes and of having a heart attack. This study builds on this and now we have evidence in humans that following intake of one portion of berries per day we can see these heart health benefits, and benefits on how we control our insulin and glucose levels. Response: The main finding of the study is that lifestyle advice with respect to mortality for patients with diabetes should not differ from recommendations for the general population. Diabetes status did not substantially influence the associations between lifestyle and mortality risk. People with diabetes may benefit more from a healthy diet, but the directions of association were similar. These results highlight that the difficulties in recognizing and diagnosing diabetes and its different stages are of minor importance with respect to healthy diet and lifestyle recommendations, because no difference in recommendations depending on the stage of the disease seems necessary. Answer: Consumption of solid fat and added sugar (SoFAS) by children exceeds recommendations, but it was not known where kids obtain these “empty calories.” Analyzing data from over 22,000 US children, we found that children consumed about 1/3 of their calories as solid fat and added sugar for foods consumed from retail food stores (including grocery stores and supermarkets), schools, or fast food restaurants in 2009-2010, despite significant decreases from 1977 to 2010 at each location. These mean levels of empty calorie intake greatly exceeded recommended amounts not just for fast foods, but also for foods consumed from schools and from stores. For all survey years, foods consumed by children from schools were higher in solid fat content than foods obtained and consumed from retail food stores. (more…)

Answer: In our prospective study of diet quality among MI survivors, we found that a higher diet quality post-MI, measured by Alternative Healthy Eating Index 2010, was associated with 24% lower death rate and 26% lower death rate from cardiovascular disease. Greater improvement of diet quality from pre- to post-MI was associated with 30% lower death rate and 40% lower cardiovascular disease death rate. In addition to reducing the bad fats intake, for example, saturated and transfat intake, MI patients also tended to reduce the good healthy polyunsaturated fats. (more…)
