Dr. Zamora-Ros:  Our study shows that diets rich on flavonoids (polyphenols ubiquitously distributed in the plant kingdom, such as in fruit, vegetables, tea, wine and chocolate), particularly flavonols, are associated with less esophageal cancer risk, especially in current smokers. Tobacco smoking causes oxidative stress, and both oxidative stress and smoking tobacco are related to increased esophageal cancer risk. Therefore, our data suggest that the possible protective mechanism of dietary flavonoids may be related to their antioxidant properties, which may not be attributed to the direct antioxidant action, but to the ability to modulate antioxidant enzymatic pathways. Dr. Pattani: We performed a meta-analysis of 16 studies that assessed the effectiveness of probiotics administered concurrently with antibiotics compared to the use of antibiotics alone. The use of probiotics among patients in these trials reduced the risk of antibiotic-associated diarrhea by almost 40% and decreased the rate of Clostridium difficile infection by 63%. On subgroup analysis, the reduction remained statistically significant for the subgroups of good quality trials, trials in which a primarily Lactobacillus-based regimen was used, and those studies which had a follow-up period of less than 4 weeks. Dr. Orlich: The main findings were these. Vegetarians, as we defined them, had reduced risk of death during the study period compared to non-vegetarians. This was true also for particular vegetarian diets including for vegans, lacto-ovo-vegetarians, and pesco-vegetarians.  Reduced risk was seen in particular for deaths related to disease of the heart, kidneys, and diabetes. Answer: At doses which could be achieved by drinking green tea on a daily basis, it has been known that green tea extract (GTE) inhibits intestinal glucose and lipid absorption when it is present in the intestinal tract. It is a reason why green tea consumption has been known to be beneficial for controlling obesity and type 2 diabetes. However, we have found that, after the intestinal absorption of GTE itself, the circulating GTE can elevate blood glucose levels by blocking cellular glucose uptake in most tissues, thereby limiting glucose utilization of the tissues. Therefore, we have tried to prolong the residence time of GTE in the intestine and block the intestinal absorption of GTE, by adding non-toxic polymer polyethylene glycol (PEG) to GTE. As a result, in 4 week treatment, the GTE+PEG regimen dramatically helps obesity-induced diabetic mice lose body weight and ameliorate glucose intolerance. Dr. Boyland: Children who were exposed to a TV commercial for Walker’s potato chips featuring a celebrity endorser showed a greater brand preference for Walker’s and consumed more Walker’s chips than a purported ‘supermarket brand’. Exposure to the celebrity endorser in a different, non-promotional context (presenting a soccer highlights TV program) also had a similar impact on brand choice and intake. This effect was not seen in response to another snack food commercial or a non-food commercial. Importantly, children did not reduce their consumption of the perceived ‘supermarket brand’ to compensate, they simply consumed more Walker’s crisps so this effect could contribute to overconsumption. Response: 1. Coffee, but not caffeine, consumption was associated with marginally increased gestational length but not with the risk of spontaneous preterm delivery. 2. Caffeine intake was consistently associated with decreased birth weight and increased odds of SGA (small for gestational age). This might have clinical implications as even caffeine consumption below the recommended maximum (200 mg/d in the Nordic countries and USA, 300 mg/d according to WHO) was associated with increased risk for SGA.
