Response: The length of a doctor’s note is taken account when determining how much a doctor or medical center is paid for a visit. However, in the digital era, a doctor can generate large amounts of text with just a few keystrokes. Given this incentive structure, we were concerned doctors’ notes might be inaccurate in certain sections of the chart that are important for billing. We used observers to determine how accurately doctors’ notes reflected the interactions between patients and physicians.

Response: Despite advances in the prevention and treatment of cardiovascular disease, it remains the number one global killer of both men and women. Patients face a variety of barriers to getting the care need, including cost and complexity of medication regimens. Innovative strategies are needed to improve the delivery of preventive care, especially when it comes to socio-economically vulnerable individuals. The polypill, a fixed-dose combination of 3 blood pressure lowering medications and a cholesterol lowering medication, may be a strategy for improving cardiovascular disease prevention. We enrolled 303 patients at a community health center in Mobile, Alabama. Half of the patients were assigned to take a daily polypill, while the other half received their usual medical care. Participants underwent a standard medical exam, blood pressure measurement, and blood cholesterol testing during their initial visit, a 2-month visit, and a 12-month visit.   • Participants in the polypill group experienced a greater reduction in both systolic blood pressure and LDL cholesterol level, as compared with participants in the usual care group. These differences translate to an approximate 25% reduction in the risk of experiencing a cardiovascular event.
  • At 12 months, adherence to the polypill regimen, as assessed based on pill counts, was 86%.
  • The vast majority of our study participants were African-American (96%), with three quarters reporting an annual income below $15,000.

Response: Executive physicals are 1 to 2-day comprehensive health assessments offering disease screening and preventive testing. Large companies can arrange for these evaluations for senior executives. They are often offered by prestigious academic medical centers, but can also be located in less formal settings like spas. They generally include a set of tests that sometimes vary based on patient characteristics. Any tests that are done in response to from findings from executive physicals are billed to health insurance companies. A single 2008 paper described executive physicals and criticized them for being non-evidence based. Since then, executive physicals have grown in popularity, but their nature and impact have not been described. We set out to describe included services and cost of executive physicals at top academic medical centers.

FDA currently does not regulate pulegone in e-cigarettes and snuff products and the health risk associated with it for the users is not well understood.

Response: For people who have already had a cardiovascular event, the benefits of aspirin generally outweigh its harms but the balance of benefits and risks is unclear in primary prevention. It was hoped that the results of three major trials published last year would determine whether or not aspirin had a role in the primary prevention of cardiovascular disease (CVD) among people at intermediate risk of CVD, but these trials recruited participants at lower CVD risk than expected. An updated meta-analysis of aspirin for the primary prevention of CVD, which incorporated the findings from these three trials, has confirmed that aspirin reduces the relative risk of CVD and increases the relative risk of bleeding. We investigated, using an individualized assessment of the absolute cardiovascular benefits of aspirin and its bleeding harms among New Zealand adults aged 30-79 years without established CVD who had their CVD risk assessed in primary care between 2012 and 2016, whether there are individuals without established CVD for whom the absolute cardiovascular benefits of aspirin are likely to outweigh its absolute bleeding harms.

In patients admitted to hospital with infection, neither variants in the PCSK9 gene nor predicted expression of PCSK9 were associated with risk of sepsis or poorer outcomes after sepsis.

Response: Based on a high-risk population in China, we have conducted a large randomized factorial-designed intervention trial (Shandong Intervention Trial) to examine the effect of short-term Helicobacter pylori (H. pylori) treatment and 7.3-year vitamin and garlic supplementation on gastric cancer. During 14.7-years’ follow-up in the trial, 2-week treatment for H. pylori resulted in statistically significant reduction in gastric cancer incidence. Results for gastric cancer mortality and for the effects of garlic and vitamin supplementation, though promising, were not statistically significant. Longer follow-up was needed to determine whether the reductions in gastric cancer incidence from H. pylori treatment would persist and lead to a demonstrable reduction in gastric cancer mortality. It also remained unknown whether vitamin and garlic supplementation would yield a statistically significant reduction in gastric cancer incidence and mortality with additionally extended follow-up. In addition, the entire spectrum of effects of these interventions needs to be understood.

Response: The idea came from my previous  investigation of compositional regulates in paintings, which showed that there is a sense of balance  between symmetry and asymmetry in a composition, such that the asymmetry composition tends to appear more dynamic and interesting, but it needs to be anchored around a symmetric point for a comfortable sense of stability. That point in adult portraits tends to be the dominant eye, placed close to the centre line, but above the centre of the painting as a whole. Selfies are a fascinating art form and the lead author has published several papers on this topic from a cognitive neuroscience perspective. One fascinating feature of selfies is that they represent pseudo-artistic productions by individuals that do not generally have academic artistic training, making it interesting to compare them to self-portraits by real artists. If you then see the same phenomena, it is likely that these are rooted in our deep nature rather than on training and cultural conventions.

Response: Men with low risk prostate cancer often enter “active surveillance” programs. These programs allow patients to defer definitive treatment (and avoid their associated side effects) until more aggressive disease is detected, if at all. Patients typically undergo a “confirmatory biopsy” 6 to 12 months after diagnosis to verify that their disease is low risk, and then undergo repeat biopsies every 1 to 2 years. These biopsies have traditionally been performed under the guidance of transrectal ultrasonography. Transrectal ultrasonography is unable to accurately visualize tumors within the prostate, necessitating that biopsy cores be obtained systematically from all parts of the prostate. MRI-ultrasonography fusion biopsy is a newer technology that has been shown to characterize biopsy findings more accurately than transrectal ultrasonography, leading to improved disease detection. This technology also allows us to visualize tumors within the prostate, and directly target these tumors during a biopsy session.

We found that men with high inflammation in late adolescence experienced increased mortality due to cancer and cardiovascular disease.

Response: In the June 18, 2019 issue of Annals of Internal Medicine, we published a systematic review and meta-analysis evaluating the cardiovascular risks of white coat hypertension (WCH; i.e. elevated office blood pressure and normal out-of-office blood pressure). The goal of the meta-analysis was to clarify previous mixed results regarding the risks of untreated WCH and treated WCH. The meta-analysis examined 27 studies – including 64,273 patients – and demonstrated that untreated WCH is associated with an increased risk of cardiovascular events (36%), all-cause mortality (33%), and cardiovascular mortality (109%) compared to normotension. This risk was strongest in studies with a mean age ≥55 years or that included participants with cardiac risk factors, such as diabetes and chronic kidney disease. We found no increased cardiovascular risk associated with treated white coat hypertension.

Response: Although one third of Medicare beneficiaries are currently enrolled in Medicare Advantage (MA), it is difficult to assess the quality of healthcare providers that serve MA beneficiaries, or to compare them to providers that serve Traditional Medicare (TM) beneficiaries. While Medicare Advantage plans are required to cover the same minimum healthcare services as TM, MA beneficiaries receive care from their plan’s network of preferred providers, while TM beneficiaries may select any Medicare-certified provider. The objective of this study is to compare the quality of Home health Agencies (HHAs) that serve Medicare Advantage and TM beneficiaries. Approximately 3.5 million Medicare beneficiaries receive home health care annually.

Response: Medical imaging increased rapidly from 2000 to 2006. The rise in imaging can be attributed to improvements in technical aspects of imaging, strong physician and patient demand, and strong financial incentives. While imaging contributes to accurate disease diagnosis and improved treatment, imaging can also increase costs and patient harms, such as incidental findings, overdiagnosis, anxiety, and radiation exposure associated with increased risk of cancer. Potential overuse of diagnostic testing has been addressed by the American Board of Internal Medicine Foundation’s Choosing Wisely Campaign and initiatives by payers to reduce imaging through payment reductions, but there remains uncertainty in the impact of these initiatives on imaging rates.The objective of our study was to evaluate recent trends in medical imaging. Our study assessed imaging from 2000 through 2016 among individuals enrolled in diverse U.S. integrated healthcare systems and among individuals residing in Ontario, Canada and assessed changes in medical imaging utilization over time by country, health system, and patient demographic factors.

Response: Every year, too many American women are faced with the challenge of dealing with a cancer diagnosis related to potentially harmful mutations of the BRCA1 and BRCA2 genes.  However, the Task Force found that there are several steps women can take to determine if they’re potentially at increased risk for BRCA gene mutations – and if genetic counseling and BRCA testing are needed. It is important to note that while some women can benefit from risk assessment, counseling, and testing, these services are not for everyone.

Response: Silent infarctions are a primary cause of strokes in the brain, but they are also common in people without a history of a stroke. Infarctions are generally only reported if they are larger (at least 3mm) and are ignored clinically if they are smaller (less than 3mm). We examined 20 years of cognitive decline among stroke free, middle-aged people with and without smaller, and larger infarctions. The comparison groups included participants as follows: those with (1) no infarctions, the reference group; (2) only smaller infarctions; (3) only larger infarctions 4) both smaller and larger infarctions
